We are seeking a Board-Certified Primary Care Physician (Family Medicine or Internal Medicine) who is passionate about delivering high-quality, compassionate care to underserved and marginalized communities within a community health/FQHC-style setting .

This role is ideal for Physicians who want to practice medicine with purpose , build meaningful patient relationships, and maintain a healthy work–life balance . The schedule is Monday–Friday , with a very manageable caseload , built-in documentation time , and strong interdisciplinary support—allowing you to focus on patient care without burnout.

Key Responsibilities

• Provide comprehensive primary care (routine, acute, same-day, and walk-in visits)

• Diagnose, treat, and manage chronic and complex conditions , including:

  • HIV/AIDS

  • Hepatitis C

  • Diabetes

  • Hypertension

  • Cardiovascular disease

  • Substance use disorders

  • Co-occurring behavioral health conditions

• Deliver trauma-informed, culturally responsive care

• Coordinate care with internal teams and external specialty providers

• Provide patient education, preventative care, and health counseling

• Maintain timely, compliant clinical documentation within the EMR (documentation time provided)

• Interpret labs and diagnostics with appropriate follow-up

• Participate in interdisciplinary meetings, case conferences, and quality improvement initiatives

• Supervise and support Advanced Practice Providers (NPs/PAs) as assigned

• Participate in limited on-call coverage on a rotating basis

Qualifications & Requirements

• MD or DO , licensed (or license-eligible) in the State of New York

• Board Certified in:

  • Family Medicine (ABFM) or

  • Internal Medicine (ABIM)

• Experience or strong interest in community health / underserved populations

• Spanish and/or French-Creole strongly preferred

• HIV certification or HIV care experience is a major plus

• Strong commitment to health equity, teamwork, and patient-centered care

• EMR proficiency
